Analysis
Based on appearances from the video, it is likely that the rigger attempted to rig a clove hitch on a carabiner to make a biner block, but instead rigged a munter hitch. The Munter hitch held initially because one of the strands was pinched in just the right way, but then released when the rappeller changed the angle of pull.
There have been two posts on the Facebook group Southwestern Canyoneering regarding this incident; one on 2/7/2013 and one on 4/16/2014.
